Fixing Old Plugs
Although over the last few years refinement and improvement in the hair transplant techniques has allowed us to create wonderful results, but we are still see many patients presenting to the office with the pluggy grafts of previous surgery. This past techniques used either the big plugs or the micro/mini type of surgry.
Plugs (link to repair ) were the method of choice many years. This technique utilized punch grafting where 10-15 hairs per graft were used. This method was a very inefficient way to utilize the donor area, let alone the unnatural appearance.
Micro-mini grafting, another older method, does not utilize the use of microscopy. Micro grafts were defined as (1-4 hairs) and mini grafts were (5-8 hairs) grafts. The size of recipient incisions has to be larger to accommodate these bigger grafts, which negatively affect the scalp circulation. Some clinics that use these out dated methodologies will mask their true nature by renaming them with fancy names such as multi-unit grafting or micro-macro grafting.
No informed patient would want micro grafts of up 4 hairs in the hairline.
When treating a patient who has had a plugs type of surgery , RAHAL has developed two extremely effective treatments that take into account any previous scarring:
- Augmentation of the existing “plugs” with new follicular unit transplants or;
- Removal of the old “plugs” (necessary when they are in the hairline) and reinsertion using our state-of-the-art methods.
In these cases, we do a thorough examination of the previous work, assess for proper circulation and develop a highly individual approach to ensure that our patients will finally achieve the best possible repair.
