Payment Terms and Booking Procedures
In order to formally book a surgery a $2000 Canadian deposit. ($1000 Non-Refundable) is needed.
The surgical balance must be paid in full, 2 weeks prior to the surgical date. Failure to do this may result in the surgery being cancelled.
Cancellation Policy: Deposit will be refunded with at least 30 days notice. ($1000 Non-Refundable)
You are required to be at the office at the prescribed time on the day of your surgery. There will be a $150 late charge for each half an hour that you are late arriving into the office. (The start time of my surgery will be given to you at your pre-operative appointment)
If your surgery is cancelled by you for any reason without the minimum 2 weeks notice – You will forfeit the $2000 deposit as a cancellation fee.
There is a $100.00 medication fee which is added to my surgical price, unless I have a drug plan.
Payment Method Options:
- Visa, MasterCard, cash, certified cheque, traveler's cheques and bank wire transfer. Please note: If you are planning to pay with your debit card make sure you are aware of your daily transaction limit.
- Medicard &CreditMedical Loans: We must receive your cheque 2 weeks in advance. This means that you must apply for the loan 5 weeks prior to the date of your surgery.
